Scheduling An Appointment |
Our courteous staff will quickly and efficiently respond to your request when you contact the Appointment Scheduling Lines at Papastavros' Associates. There are 4 Full Service Sites and numerous Satellite Sites throughout the State of Delaware that provide the highest level of Medical Diagnostic Radiological Services.
An appointment may be scheduled by either you or your physician. You will need a Referral Form from your doctor, regardless of how the appointment is scheduled.
Your insurance company may require that you get a Pre-Approval before your tests. Please contact your provider or HMO for authorization.

When you schedule your appointment, you will receive information about any preparations that may be necessary for your study.
Please be sure to bring the following:
- X-rays:
Please bring X-rays or mammograms performed at any office or hospital other than those created at any of Papastavros' Associates Radiology sites.
- Medications:
Please bring a list of all medications and dosages, including all over the counter medicines you are currently taking.
- Insurance information:
Please bring your current insurance card or other insurance information to your appointment. In order to properly bill your insurance carrier(s) for any tests, please provide to us the name of your current insurance carrier (company), their complete mailing address, your policy's group number, and your personal identification number.
If you are covered by Medicare or Medicaid, please bring your cards or coverage information to your appointments. If your insurance carrier requires a referral form, please bring it with you to your appointment.
Please remember that Papastavros' Associates follows the most strict HIPPA Guidelines.
- Form of Payment:
You may be asked to pay for services not covered by insurance. Please bring cash, check, or major credit card (American Express, Visa or Mastercard).
